Calderdale PCNs’ Personalised Care Team latest newsletter

The Calderdale Primary Care Network (PCN) Personalised Care team work across and with GP practices within the five PCNs in Calderdale, which con-sist of, Central Halifax PCN, Lower Calder Valley PCN, Upper Calder Valley PCN, North Halifax PCN and Calder & Ryburn PCN. The personalised care team consists of Social Prescribing Link Workers (SPLWs) and Care Coordi-nators.
The SPLWs and Care Coordinators that form the personalised care team are employed by Pennine GP Alliance (PGPA) and North Halifax PCN.
Personalised care means people have choice and control over the way their care is planned and delivered. It is based on ‘what matters’ to them and their individual strengths and needs.
By developing the roles of the SPLWs and Care
Coordinators, Calderdale PCNs are empowering their patients.
